What is FTM Top surgery?
Female to male top surgery (FTM) is a gender confirmation breast procedure that helps patients feel more comfortable in their skin. The procedure helps patients align with who they are inside. FTM top surgery reduces a patient’s breast size and creates a masculine chest.
Surgical methods used to achieve a patient’s desired results include liposuction and excision of breast tissue. Liposuction is good for patients that have a small chest with no sagging skin. For patients with larger breasts, excisional methods are typically used. These methods include inverted-t, double incision, keyhole incision, buttonhole incision and periareolar incision.
The most common methods are:
- Inverted-T: This method removes extra skin and keeps the nipple/areola attached.
- Double Incision: A horizontal incision is made on the lower border of the pectoral muscle, then fatty breast tissue is removed. Each nipple may be resized, if necessary.

What to expect post-procedure:
- To be off work or school for about 1 week.
- To wear a compression garment over the surgical area for about 6 weeks.
- Depending on the patient, drains may be placed.
- Scarring will be visible, but eventually fade overtime.
